In this rewarding position you are working directly with clients in our acute care facility. Proing resources for care, assisting the therapeutic team and encouraging healthy choices as your clients transition to outpatient therapy and treatment. (Fulltime 8a-5p. WellStar)

What we re looking for:

• Bachelor’s Degree in social sciences field
• Excellent 5 year driving record & reliable transportation
• Excellent Oral and Written Communication Skills
• State of Alabama Adult Case Management Training
• Knowledge of Community Lay-out (Madison County Area)
• Knowledge of Community Agencies and Resources
• Personal Automobile Liability Insurance in the amounts of $100k, $300k, $50K

What you ll be doing:

• Proe in patient case management services
• Proe direct services to mentally ill clients to address needs which may have been unmet as well as information about continuum of care
• Proe follow-up progress as per need of client to treatment team members
• Proe Basic Living Skills training when necessary
• Assist clients with obtaining medication
• Monitor medicationpliance, maintain awareness ofmon e effects, and ensure client is taking medication as directed on a regular basis
• Proe traditional case management services as well as collateral services such as transportation to appointments, referral agencies, etc.
• Maintain high quality client documentation and records management utilization electronic charting
• Monitor the care and services clients receive in reential setting
• Develop and implement Indiual Service Plan which consist of goals, resources, and interventions
• Revise client’s goals once met, or progress as they occur
• Assist mentally ill clients through crisis situations and/or arranging for the provision of such assistance by other professional or personal caregivers
• Perform inental care to families of the mentally ill and assist those family members in understanding the mental illness

Education Required

• Bachelors or better in Counseling, Psychology, Social Work, etc
