Job Summary

Facilitates the coordination of services for developmentally delayed children who qualify for Early Intervention. Assists child/family in development of the Individualized Family Service Plan. Provides supportive counseling and crisis intervention to child/family. Advocates for children on state and local level.

Essential Job Functions

• Maintains a caseload of approximately 80-95 families and completes all mandated work.
• Handles hiring process for all new Service Coordinators and works jointly with the Program Coordinator and Administrative Senior Service Coordinator (ASSC) to orientate new Service Coordinators.
• In charge of monthly tracking of Annual/Exit BDI lists. Works with the Hudson County Targeted Evaluation Teams and Service Coordinators to ensure adherence to NJEIS Policy and best practices.

• As necessary, oversees twice-weekly broadcast of eligible children and related duties.
• Participates in program development, makes recommendations to Director/Program Coordinator for program growth.
• Participates in all department, hospital and outside agency/institution committees as assigned/delegated.
• Prepares/maintains unit readiness for inspections, surveys, ensures program compliance with all standards.
• Works jointly with Director/Program Coordinator to meet department s internal goals and strategic metrics.
• Takes responsibility for professional growth.
• This position requires, at a minimum, at least one-year prior experience as a Service Coordinator; preferred LSW in the State of New Jersey.

Formal Education and Job-Related Experience

• Master’s degree in social work (LSW) from an accredited program preferred but not required and one year experience in public health or with children who are chronically ill or disabled.
• BA or BS Education and Teacher of the Handicapped Certification, BA Psychology welcome to apply.
• One-year experience in public health or with children who are chronically ill or disabled.
• One-year experience as a Service Coordinator.

Job Specific Contacts

Continual contact with children and their families. Frequent contact and meetings with the Regional Early Intervention Collaborative – Hudson Counties. Occasional contact with NJ Special Child Health Services. Contact with Service Coordinators throughout the state, as needed. Frequent contact with professionals form resource/referral network and EI Programs.

Physical Demands

• A minimum of one-year experience working with children and families.
• Must have availability of personal vehicle and able to travel by car as needed to attend meetings and conduct home visits.
• Ability to work remotely and in Office location.
